BackgroundCommunity engagement within biomedical research is broadly defined as a collaborative relationship between a research team and a group of individuals targeted for research. A Community Advisory Board is one mechanism of engaging the community. Within genomics research CABs may be particularly relevant due to the potential implications of research findings drawn from individual participants on the larger communities they represent. Within such research, CABs seek to meet instrumental goals such as protecting research participants and their community from research-related risks, (...) as well as intrinsic goals such as promoting the respect of participants and their community. However, successful community engagement depends on the degree to which CABs legitimately represent and engage with communities targeted for research. Currently, there is little literature describing the use of CABs in genomics research taking place in developing countries, and even less in the field of genomics research relating to mental illness. The aim of this article is to describe and consider the contributions made by a researcher-driven, population-specific CAB in a genomics of schizophrenia research project taking place in South Africa, from the perspective of the research team.DiscussionFour broad discussion topics emerged during the CAB meetings namely: 1) informed consent procedures, 2) recruitment strategies, 3) patient illness beliefs and stigma experiences, and 4) specific ethical concerns relating to the project. The authors consider these discussions in terms of their contributions to instrumental and intrinsic goals of community engagement.SummaryThe CAB gave valuable input on the consent processes and materials, recruitment strategies and suggested ways of minimizing the potential for stigma and discrimination. All of these contributions were of an instrumental nature, and helped improve the way in which the research took place. In addition, and perhaps more importantly, the CAB made a unique and important contribution relating to intrinsic functions such as promoting the respect and dignity of research participants and their community. This was particularly evident in ensuring sensitivity and respect of the community’s traditional beliefs about schizophrenia and its treatment, and in this way promoting a respectful relationship between the research team and the participants. Cell line immortalisation is a growing component of African genomics research and biobanking. However, little is known about the factors influencing consent to cell line creation and immortalisation in African research settings. We contribute to addressing this gap by exploring three questions in a sample of Xhosa participants recruited for a South African psychiatric genomics study: First, what proportion of participants consented to cell line storage? Second, what were predictors of this consent? Third, what questions were raised by participants during (...) this consent process? 760 Xhose people with schizophrenia and 760 controls were matched to sex, age, level of education and recruitment region. We used descriptive statistics to determine the proportion of participants who consented to cell line creation and immortalization. Logistic regression methods were used to examine the predictors of consent. Reflections from study recruiters were elicited and discussed to identify key questions raised by participants about consent. Approximately 40% of participants consented to cell line storage. The recruiter who sought consent was a strong predictor of participant’s consent. Participants recruited from the South African Eastern Cape, and older participants, were more likely to consent; both these groups were more likely to hold traditional Xhosa values. Neither illness nor education were significant predictors of consent. Key questions raised by participants included two broad themes: clarification of what cell immortalisation means, and issues around individual and community benefit. These findings provide guidance on the proportion of participants likely to consent to cell line immortalisation in genomics research in Africa, and reinforce the important and influential role that study recruiters play during seeking of this consent. Our results reinforce the cultural and contextual factors underpinning consent choices, particularly around sharing and reciprocity. Finally, these results provide support for the growing literature challenging the stigmatizing perception that people with severe mental illness are overly vulnerable as a target group for heath research and specifically genomics studies. Collective action processes in complex, multiple-use common-pool resources (CPRs) have only recently become a focus of study. When CPRs evolve into more complex systems, resource use by separate user groups becomes increasingly interdependent. This implies, amongst others, that the institutional framework governing resource use has to be re-negotiated to avoid adverse impacts associated with the increased access of any new stakeholders, such as overexploitation, alienation of traditional users, and inter-user conflicts. The establishment of “platforms for resource use negotiation” is a (...) way of dealing with complex natural resource management problems. Platforms arise when stakeholders perceive the same resource management problem, realize their interdependence in solving it, and come together to agree on action strategies for solving the problem (Röling, 1994). This article sets the scene for a discussion in this Special Issue about the potential of nested platforms for resource use negotiation in facilitating collective action in the management of complex, multiple-use CPRs. The article has five objectives. First, we define “collective action” in the context of this paper. Second, we discuss the importance of collective action in multiple-use CPRs. Third, we introduce the concept of platforms to coordinate collective action by multiple users. Fourth, we address some issues that emerge from evidence in the field regarding the role and potential of nested platforms for managing complex CPRs. Finally, we raise five discussion statements. These will form the basis for the collection of articles in this special issue. (shrink)

What are the conditions required for becoming better human beings? What are our limitations and possibilities? I understand “becoming better” as a combined improvement process bringing persons “up from” a negative condition and “up to” a positive one. Today there is a tendency to understand improvement in a one-sided way as a movement up to the mastery of cognitive skills, neglecting the negative conditions that can make these skills mis-educative. I therefore tell six stories in the Western tradition about conditions (...) for a combined improvement process. The first three stories belong to our cultural ABC: an Aristotelian story about moral wisdom which brings people up from being enslaved by passions and up to a good life of virtues; a Biblical story about God’s word bringing listeners up from a self-centred life and up into creative work as God’s fellow workers, and a short Cave story by Plato about liberation—up from living by common illusions and up to enlightenment from what is perfectly good. The subsequent three stories interpret and actualise these basic stories in different ways: a story about moral wisdom and divine love (Thomas Aquinas), a story about individual freedom and rationality (Immanuel Kant), and a story about the love that builds us up as equal human beings (Søren Kierkegaard). These stories may directly guide us adults—and indirectly the children and youth who learn from our examples—when we struggle to become better human beings. (shrink)

Both in formal situations (as school teachers, football trainers, etc.) and in many, often unpredictable informal situations (both inside and outside institutions)—adults come close to children. Whether we intend it or not, we continually give them examples of what it is to live as a human being, and thereby we have a pedagogical responsibility. I sketch what it could mean to let ourselves “be built up”, in a Kierkegaardian sense, on the foundation of unconditional love, presupposing that this love is (...) possible for all human beings. Kierkegaard’s Upbuilding discourses invite each reader to engage in a dialogue with the possibilities in the text. Thereby the reader may become aware of his or her present situation in life and see possible alternatives. These discourses or “talks” (taler in Danish) exemplify a manner of indirect communication which perhaps may be transferred to encounters with works of art in general: How could I let examples in literature, pictures, films and music invite and challenge me—to ask myself who I am right now and who I ought to be? My aim is to present an alternative to the instrumental advices that adults are given today. I attempt to clarify the leading concept “upbuilding examples”, sketch the difference between upbuilding, education and Bildung, refer to works of art that seem to have upbuilding possibilities, and consider why upbuilding examples should be studied and how they could be studied in small self-governed groups of adults. Children as learners need adults who love them, even when the children are unable to give anything in return. Furthermore, adults should be able to make wise judgements concerning what is good for the children. The clarification of these principles and of their educational import has to start within our own cultural tradition. Agape (unconditional love, neighbour-love or charity) is a basic concept in the Christian tradition. Phronesis (moral wisdom, practical judgement or prudence) has a key position in the Aristotelian (...) tradition. In his Summa TheologiaeThomas Aquinas has combined these traditions and ethical concepts, with agape (in Latin caritas) as the commanding concept. The article will explore some key challenges and productive resources revealed by this combination for today's education and upbringing. (shrink)

Despite the effort on DSM-5 and ICD-11, few appear satisfied with these classification systems. We suggest that the core reason for dissatisfaction is expecting too much from them; they do not provide discrete categories that map to specific causes of disease, they describe clinical syndromes intended to guide treatment choices. Here we review work on anxiety and anxiety disorders to argue that while clinicians draw a pragmatic distinction between normal and abnormal emotions based on considerations such as severity and duration, (...) understanding the evolutionary origins and utility of the emotions, including the adaptive value of adverse emotions, is key for formulating comprehensive assessments of an individual patient’s symptoms and for providing a conceptual foundation for pharmacotherapy, psychotherapy, and public health. All of our information about the world is derived from the function of our senses, and thus they are the principal source of all our knowledge. This was recognized explicitly by early Greek philosophers, remained an important point of discussion for nineteenth‐century philosophers, and continues to be a key issue for present‐day philosophers, psychologists, and neuroscientists. It is a key issue in cognitive science because, by initiating the processes that store and evaluate information, sensory information transmission can be considered a (...) cognitive initiator. In a very real sense, who we are and how we see the world are the result of the experiences that are mediated by our sensory systems. These are some of the reasons why the study of sensory systems has occupied such a prominent place in modern cognitive neuroscience. (shrink)

Grafts of embryonic neural tissue into the brains of adult patients are currently being used to treat Parkinson's disease and are under serious consideration as therapy for a variety of other degenerative and traumatic disorders. This target article evaluates the use of transplants to promote recovery from brain injury and highlights the kinds of questions and problems that must be addressed before this form of therapy is routinely applied. It has been argued that neural transplantation can promote functional recovery through (...) the replacement of damaged nerve cells, the reestablishment of specific nerve pathways lost as a result of injury, the release of specific neurotransmitters, or the production of factors that promote neuronal growth. The latter two mechanisms, which need not rely on anatomical connections to the host brain, are open to examination for nonsurgical, less intrusive therapeutic use. Certain subjective judgments used to select patients who will receive grafts and in assessment of the outcome of graft therapy make it difficult to evaluate the procedure. In addition, little long-term assessment of transplant efficacy and effect has been done in nonhuman primates. Carefully controlled human studies, with multiple testing paradigms, are also needed to establish the efficacy of transplant therapy. (shrink)

ABSTRACTSchumpeter’s theory of socialism pivots on his response to Ludwig von Mises’s claim that rational economic calculation is “impossible” in a socialist economy. Mises held that because socialism eliminates market prices for the means of production, it is impossible under socialism to know the relative scarcities of productive inputs, and thus to determine rationally which of any number of technologically feasible production projects to pursue. Schumpeter appears to assume away Mises’s epistemic concerns about socialism by contending that it is theoretically (...) possible to determine which goods should be produced in a socialist economy if all the relevant data are known—which begs the question Mises had asked. Did Schumpeter really commit such an elementary error of interpretation? Or was the appearance of doing so part of his attempt to communicate to the reader of Capitalism, Socialism and Democracy a host of reservations about the likely consequences of socialism in practice, even as he ostentatiously praised its feasibility in principle? Despite some clinical promise, using fetal transplants for degenerative and traumatic brain injury remains controversial and a number of issues need further attention. This response reexamines a number of questions. Issues addressed include: temporal factors relating to neural grafting, the role of behavioral experience in graft outcome, and the relationship of rebuilding of neural circuitry to functional recovery. Dispositional forgiveness is positively associated with many facets of wellbeing and has protective implications against depression and anxiety in adolescents. However, little work has been done to examine neurobiological aspects of forgiveness as they relate to clinical symptoms. In order to better understand the neural mechanisms supporting the protective role of forgiveness in adolescents, the current study examined the middle frontal gyrus, which comprises the majority of the dorsolateral prefrontal cortex and is associated with cognitive regulation, and its relationship to (...) forgiveness and clinical symptoms in a sample of healthy adolescents. In this cross-sectional study, larger MFG volume was significantly associated with higher self-reported dispositional forgiveness scores and lower levels of depressive and anxiety symptoms. Forgiveness mediated the relationship between MFG volume and both depressive and anxiety symptom levels. The mediating role of forgiveness in the relationship between MFG volume and clinical symptoms suggests that one way that cognitive regulation strategies supported by this brain region may improve adolescent mental health is via increasing a capacity for forgiveness. The present study highlights the relevance of forgiveness to neurobiology and their relevance to emotional health in adolescents. Future longitudinal studies should focus on the predictive quality of the relationship between forgiveness, brain volume and clinical symptoms and the effects of forgiveness interventions on these relationships. Falk's hominin mother-infant model presupposes an emerging infant capacity to perceive and learn from afforded gestures and vocalizations. Unlike back-riding offspring of other primates, who were in no need to decenter their own body-centered perspective, a mirror neurons system may have been adapted in hominin infants to subserve the kind of (m)other-centered mirroring we now see manifested by human infants soon after birth.

Ensuring the trustworthiness of autonomous systems and artificial intelligence is an important interdisciplinary endeavour. In this position paper, we argue that this endeavour will benefit from technical advancements in capturing various forms of responsibility, and we present a comprehensive research agenda to achieve this. In particular, we argue that ensuring the reliability of autonomous system can take advantage of technical approaches for quantifying degrees of responsibility and for coordinating tasks based on that. Moreover, we deem that, in certifying the legality (...) of an AI system, formal and computationally implementable notions of _responsibility_, _blame_, _accountability_, and _liability_ are applicable for addressing potential responsibility gaps (i.e. situations in which a group is responsible, but individuals’ responsibility may be unclear). This is a call to enable AI systems themselves, as well as those involved in the design, monitoring, and governance of AI systems, to represent and reason about who can be seen as responsible in prospect (e.g. for completing a task in future) and who can be seen as responsible retrospectively (e.g. for a failure that has already occurred). To that end, in this work, we show that across all stages of the design, development, and deployment of trustworthy autonomous systems (TAS), responsibility reasoning should play a key role. This position paper is the first step towards establishing a road map and research agenda on how the notion of responsibility can provide novel solution concepts for ensuring the _reliability_ and _legality_ of TAS and, as a result, enables an effective embedding of AI technologies into society. This masterful collaboration excels in many areas, including in-depth prosopographical analysis and thorough integration of the local seal practice into the orbit of textual analysis. This volume sets a standard of text edition and analysis that dwarfs the achievements of the students (...) of Nuzi from the first two-thirds of the twentieth century and even since. Nevertheless, the raison d’être of this heterogeneous group of texts in the context of their storage in temples remains enigmatic. (shrink)

Haney; xiii & 269 pp. University Park: Pennsylvania State University Press, 1993, $35.00.To the English Romantic poets, David Haney notes, the world seemed to have died at the hands of Enlightenment rationalism by being made merely a referent of transpicuous representational sign-systems. One of their fundamental projects was to reanimate it, (...) and Wordsworth committed himself to an animating poetics, incarnational in concept. Haney quotes the Wordsworthian incarnational locus classicus, the third “Essay Upon Epitaphs”: “If words be not... an incarnation of the thought but only a clothing for it, then surely will they prove an ill gift;... Language, if it do not uphold, and feed,... is a counter-spirit unremittingly and noiselessly at work to derange, to subvert, to lay waste, to vitiate, and to dissolve” (p. 1). He conjoins Wordsworth’s statement in a note to “The Thorn” that words should seize us “not as symbols of the passion, but as things, active and efficient, which are themselves part of the passion” (p. 24). “Incarnational” is not further defined directly, though we are told that it characterizes a poetic practice, “a philosophically and theologically important concept embedded [in our cultural] heritage,” and a theoretical orientation that acts “against the very idea of a ‘theory’” (p. 4). Haney draws principally from Gadamer, Lévinas, Cavell, Geoffrey Hartman, and Charles Taylor to explore through philosophical argumentation the ways in which Wordsworth incorporated, elaborated, problematized, and even betrayed these aspects of the incarnational process in his work.One of Haney’s concerns is to show how reading through the lens of incarnational poetics helps us see that Wordsworth incorporated historicity rather than suppressing it, as he is often accused of doing these days. Another is to demonstrate that deconstructive analysis, new-historical critique, and the secularized-Protestantism approach of M. H. Abrams are each inadequate to account for the hermenuetics of this poetics, because it resists and finesses the drive to create the transparent representational structures on which these critics focus. In his incarnational mode, his poetic thought aims not at realizing allegorized abstract truth but at presenting kerygmatically what is instantiated by incarnation as event; thought becomes materially existent without being changed into something else. Incarnational poetics thus acts to counter the binary oppositions (presence/absence, body/spirit, divine/human, etc.) on which both Enlightenment and Romantic discourse relied (the latter reversing the values), and defeats the binarism-dependent drive toward pure self-consciousness that always gets entangled in solipsism and semiological webs. Incarnating thought opens it to the historical process in the sense of Gadamer’s wirkungsgeschichtliches Bewusstsein: incarnation is not embodiment or an “ideal effacement of the difference between word and referent, but rather a process of spirit becoming event”; and words thereby become “things and events... which must enter the realm of mortality” (p. 19). These connections allow him to enlarge brilliantly our understanding of The Prelude, Book 5, and the “Lucy poems.” Here words achieve visionary power not by a symbolic link to transcendence but “by allowing themselves to be interrupted by that which is absolutely other, and therefore ‘infinite’ in Levinas’ sense” (p. 87). Haney extends this and related insights in a chapter on the final books of The Prelude that shows, among other things, how its figural structures for the autobiographical self begin to break down and are replaced by tropological performatives. Today's personable, sanitized images of human embryos and fetuses require an audience that is literally and metaphorically distanced from dead specimens. Yet scientists must handle dead specimens to produce embryological knowledge, which only then can be transformed into beautiful photographs and talking fetuses. I begin with an account of Gertrude Stein's experience making a model of a fetal brain. Her tactile encounter is contrasted to the avant-garde artistic tradition that later came to dominate embryo imagery. This essay shows the (...) embryo visualizations portrayed in a contemporary coffee-table book about gestational development to be a remarkable political achievement predicated, in part, on keeping hidden the unsavory details of anatomical technique that transform dead specimens into icons of life. Reimagining “humanity” is a powerful aim of the educational philosophy of Zak Stein. A relative new-comer to the field of Education and philosophy, Stein has in the last decade or so shown himself to be a potent “developmentalist,” and visionary of a “metamodern metaphysics” for reconfiguring how societies understand learning, new forms of education, and most interestingly he situates his work within a metaphysics of Love. Stein poignantly calls for a “return” to important past wisdom in transforming (...) learning but he does so from a critical integral perspective in ways that are unique in educational writing today. The author appreciates and demonstrates Stein’s awareness of the meta-context of the Anthropocene and “crises” as a critical basis for any design of future education. He particularly focuses this introductory technical paper on Stein’s views on fear and its essential core role in shaping human evolution and thus our ways of learning and designing curriculum. The author adds a brief critique of how Stein’s work could be improved in terms of a holistic-integral fear management/education in this discussion of a new metaphysics of love for education in general. Through an analysis of particular sections in Proust's A la recherche du temps perdu and several pieces by Stein, I examine how their search for bodily presence fosters the development of new styles of writing as the perceptual responses of both authors override the function of the narrator. The dissertation analyzes Husserl's phenomenological ideas on motion and the body and how they are further developed in France by Merleau-Ponty. I then use their phenomenological research in order to expand upon (...) notions of physicality in both literary authors. The action of the author writing, of the body "making marks" on paper, are qualities of the physicality of writing. In Proust the visceral is a key to the mnemonic: the body 'recalls' memory to us, where we then find bodily experiences---not just images of memories, but the physical feeling of what has been and what remains physical, I demonstrate how the episodes in which Proust encounters involuntary recollection are places of physical activity; some physical presence has to occur, or some physical activity must be taking place in order for memory to become present. Stein's experiments with language are renditions of how the author makes sense of her body and the world through perception. Her language actually 'stands in' for the spoken voice of a narrator who records a world in flux rather then narrating a story. (shrink)

The wife of a childhood friend was a close match and gave him one of her kidneys. Before his transplant, aerobic exercise was difficult; a few months after transplant, (...) Slakter could ride his bike, hike, and work out with a personal trainer. He no longer had to weigh food or gauge his potassium consumption, a concern for people with kidney failure. For Slakter, “life post-transplant has been a significant improvement.”Individuals or family members of those who have received an organ transplant from a living or deceased donor share their stories in this issue of Narrative Inquiry in Bioethics. The symposium is titled “Receiving the Gift of Life: Stories from Organ Transplant Recipients.” Organ transplantation can extend an individual’s life by decades. It can also greatly improve an individual’s quality of life, as was Slakter’s experience. At the same time, organ transplantation is expensive and stressful. Recipients can experience complications, including organ rejection, and there are few appealing alternatives.After her heart transplant, author Leilani R. Graham required four open-heart surgeries, ECMO, and an intra-aortic balloon pump. She experienced delirium, necrotizing pneumonia, and severe muscular atrophy that required her to re-learn how to walk. She struggled with weight gain, explosive mood swings, hair loss, and was terrified that her body would reject the new heart. For Graham, “[Transplant] was not the miracle [she] was hoping for.” Graham says she felt unprepared for and inadequately informed about what to expect after her transplant, which left her “battered, beaten, and bruised” with a 12-inch line from neck to navel and “bullet holes” up and down her torso from the chest tubes. When reflecting on her experience post-transplant, Graham says, “It wasn’t the physical trauma that hurt the most. It was that no one seemed surprised [by the aftereffects of transplant].” Seemingly no one, that is except Graham, who calls on transplant providers to “prioritize empathy.”The symposium editors are Jason T. Eberl and Tristan McIntosh. Three commentary articles, written by Macey Leigh Levan, Heather Lannon, and Vidya A. Fleetwood, Roslyn B. Mannon & Krista L. Pearlman. The relief of suffering is a primary goal of medicine. However, the authors note that clinicians often miss or ignore suffering because they focus on the causes rather than the experience of suffering, or the subjective nature of suffering makes it difficult to discuss. The authors believe that “the key to communicating about suffering lies in finding [End Page vii] the right kind of words to describe the experience” and say that figurative language offers a promising approach. In their research, the authors performed qualitative analysis on 52 works of literature and identified 254 excerpts of figurative language with themes of non-physical or psychological suffering.
